Your physician evaluates your medical situation and, if appropriate, completes and signs the OMMA Physician Recommendation Form during the visit. Apply online at the OMMA MedPortal at medportal.omma.ok.gov. Upload your physician recommendation, proof of Oklahoma residency, proof of identity, and a digital photo. Pay the $100 application fee (or $20 reduced fee if eligible). The signed recommendation form must be dated within 30 days of submission.
OMMA processes patient applications within 14 business days. Upon approval, your physical card is mailed via USPS first-class mail. Your approval email or MedPortal status can serve as valid patient ID at any licensed Oklahoma dispensary while your card is in transit.

The program is administered by the Oklahoma Medical Marijuana Authority (OMMA) — established by State Question 788 in 2018.
Oklahoma's medical marijuana program does not use a state-approved list of qualifying conditions. Instead, the program uses a practitioner-discretion model: an authorized Oklahoma physician evaluates the patient's medical situation and, exercising professional judgment, decides whether to complete and sign the OMMA Physician Recommendation Form. The signed recommendation must be dated within 30 days of the patient's MedPortal application submission. The standard OMMA patient application fee is $100 plus a $4.30 credit card processing fee. Oklahoma offers a reduced fee of $20 plus $2.50 processing for applicants who provide acceptable proof of Medicaid (SoonerSelect), Medicare enrollment, or 100% disabled veteran status. Fees are non-refundable and paid directly to OMMA through the MedPortal using Visa, MasterCard, or Discover.
Per 63 O.S. § 420, a licensed patient may possess up to 3 ounces on their person, 8 ounces at home, 1 ounce of concentrate, 72 ounces of edibles, plus 6 mature plants and 6 seedlings for home cultivation. Oklahoma also codifies patient protections — non-discrimination in employment, housing, custody/visitation, and medical care — under 63 O.S. § 425 and 63 O.S. § 427.8.
